Body Building Irony

Men who wish to optimize their muscle mass through the use of supplemental androgens, testosterone, anadrol or others on the list may cause themselves a physical change that works against those goals.  The aromatization of male hormone supplements result in the development of male breast tissue, called gynecomastia.  The internet is rife with supplement “antedotes”, other pills that profess to reverse the breast development effects of steroid use.  None work. Gynecomastia is common in men from the time of adolescence on, and is frequently seen in the relatives of those who seek help.  The permanent solution is surgical, and the most ideal method to undertake it is with Vaser liposelection, a one hour outpatient procedure.  Vaser ultrasound has been shown to stimulate the best skin contraction as well.  Occasional patients also have a very fibrous, uncomfortable “hockey puck” of breast glandular tissue that additionally requires a small incision at the nipple to remove it.  The Vaser selectively liquefies the fat component, markedly reducing the amount of surgery required to permanently remove the fibrous remnant.

No Sweat!

Many people are afflicted with excess perspiration in the armpits which is very disturbing to them on many levels.  The basis is profuse production from the apocrine sweat glands found in the hair-bearing region under the arms, often made worse by exertion or stress.    Topical agents are not very effective and have side effects that preclude long term use.  Botox administered in the office works beautifully for 4 to 6 months but has to be re-administered over the long term. (Many brides and bridesmaids have this done a few weeks or months before the wedding!)  Recent research has demonstrated a long term effect of using the ultrasonic energy of Vaser to effectively destroy the apocrine sweat glands found in the axillary region.  The patient requires no general anesthesia.  The Vaser is administered through two ¼-inch incisions in a minor surgery setting.  You can shower the next day and resume work in about 48 hours.  It works immediately!  You will enjoy long term reduction in perspiration and embarrassment!

Crooked Finger Fix 

Recently the FDA approved an injection therapy for Dupetryn’s contracture.  The collagenase enzyme has been around for a long time experimentally.  The clinical use of this biochemical agent involves an injection performed in the office to digest the cord collagen.  Patients who have contracture at the MCP (the knuckle) or at the PIP (the first joint) from Dupetryn’s fibrosis are candidates for the injection therapy.  The fibrosis in the palm is not as responsive to injection therapy and may require excisional surgery using tourniquet anesthesia.  Patients who have the finger contracture can be injected monthly up to three times.  The patient is seen the next day for a mobilization maneuver to hopefully break the cord.  Tendon rupture is a small risk of the injection, because it contains high collagen content and is in close proximity to the fibrotic cord in the finger.  I am credentialed in the safe use of Xiaflex™ collagenase injection.  The procedure is not cosmetic.

PS.   This injection therapy is ideal for elderly patients who might be on anti-platelet or Coumadin agents who don’t want to have surgical intervention.

Rhinoplasty, Septoplasty Focus of Attention

A common condition that worsens with age is closing down of one or both nasal passages.  Age old trauma or just your heredity contribute to septal deviation and airway obstruction.  The old trauma may take a straight septum and make it crooked years later.  Of course, the cartilages of the nose actually do get bigger as we age.  This may make a nose look larger or wider, with a bump that wasn’t there in your 20’s.  Snoring is improved with septoplasty and turbinate treatment.  If that is your history, then you may be a candidate for corrective nose surgery paid for by your insurance!   “How it breathes” is covered by your plan usually.  “How it looks” is considered cosmetic.  These problems are usually fixed in a single operation called Septo-rhinoplasty.  The correction of the airway is performed in the same operation as the cosmetic nose re-shaping procedure.  The recovery for both is similar to that required for the septoplasty.  Packing of the nose is rarely required.  The open rhinoplasty technique allows complete visualization of the nose and septal structures for rehabilitation of function and form.
