Driving Directions

From Interstate 90 (Cleveland)

• Route 2 (East) passing the Route 306/Reynolds Rd. and the Route 615/Center Street exits.
• Exit at Heisley Road turning right (South).
• Continue South on Heisley Road crossing Jackson Street.
• About 1/2 a mile from Jackson Street is the MENTOR MEDICAL CAMPUS. Turn left.
• Enter through the north entrance of the Physicians Office Pavillion to arrive at our office.

From the East using Interstate 90

• Follow I-90 west to the Route 44 (Chardon/Painesville) exit and turn right (North).
• Take Painesville exit (first exit) off Route 44.
• Turn left onto Route 84.
• Turn right onto Old Johnnycake (Zapp's gas station on your left).
• Turn left onto Mentor Avenue/Route 20.
• Turn right at first traffic signal into the Lake Hospital System MENTOR MEDICAL CAMPUS.
• Enter through the North entrance of the Physicians Office Pavillion to arrive at our office, located in Suite 100.

From I-71 South and I-77 South

• Follow I-71 or I-77 North to I-271 North (toward Erie, PA).
• Continue on I-271 to I-90 East.
• Exit at 615/Center Street.
• Turn left (North) onto 615.
• Turn right onto Mentor Avenue/Route 20 (second light).
• Continue on Mentor Avenue for 2.3 miles
• Turn left into the Lake Hospital System MENTOR MEDICAL CAMPUS.
• Enter through the North entrance of the Physicians Office Pavillion to arrive at our office, located in Suite 100.
